The Change Champions Programme is part of the North East Lincolnshire Building Choice and Releasing Community Capacity Programme and is designed to provide staff and the community, protected space to work through the development of new services to address lack of provision in some much challenged communities.
Working across the area of North East Lincolnshire which has three small towns and a larger conurbation of villages and rural settlements, the Building Choice Releasing Community Capacity Programme is assets based and has the aim of improving health and wellbeing.
The area has an increasingly ageing population with higher than average levels of long term conditions. Deprivation is masked as there are areas of affluence surrounding areas of serious deprivation in fact some of the wards in North East Lincolnshire suffer some of the highest levels of deprivation in the UK.
The Building Choice Releasing Community Capacity Programme is a two – year programme which commenced in October 2012. It has been designed to provide a catalyst for local communities and services to work together to address the issues created by an increasingly ageing population with increased levels of illness and deprivation. The programme recognises and builds on the excellent work already underway in the area and it adds value to these by ensuring impact is measurable and what works is sustainable. The programme is designed and delivered by Unique Improvements in conjunction with KHC Consulting Ltd and is designed to work on three levels,

- Quality Improvement
- Assests Based Community Development
The Change Champion Programme is supported by a skills development programme which offers free development sessions to anyone working or living in North East Lincolnshire on a range of topics including ABCD, Engagement, Social Marketing, Measurement, Developing business models and many more.
