STRENGTHENING INTEGRATED CARE – A multi disciplinary approach to improving care for people with long-term conditions.

UNI are supporting SunWest Practice Based Commissioning Consortia to implement the Castlefields Integrated Care Model to improve care and quality of life for patients aged over 75 who have multiple long term conditions. The model enables a more individual level of support for this vulnerable group of patients, with the intention of preventing hospital admissions and reducing bed days through hospital in reach. Supporting World Class Commissioning

The quality of commissioning is positively associated with efficiency levels (Health Foundation report 2010, Commissioning Health: A comparison of English Primary Care Trusts).  In the current financial climate improving commissioning is of critical importance and with the second round of World Class Commissioning Assurance Frameworks panel assessments due in the summer, PCTs should be focusing on improving commissioning as a priority. Social Marketing in NHS Sefton

UNI are working with NHS Sefton to support a range of screening activity. The work will involve engaging with local communities to gather insight to develop social advertising materials and recommended approaches.  The work will cover cervical screening amongst young women, bowel cancer screening and diabetic retinopathy screening.
